For your convenience, you can click on the forms below and submit your paperwork online or print your paperwork  and bring to your first session.  Or, you may arrive 30 minutes prior to your first session to fill out the appropriate paperwork.  It is highly helpful to fill out your paperwork prior to coming for counseling.

     4. Click on your browser's back to return to this page and fill out the next form
